Medical Assistance

Medicaid is a health care plan that is both state and federally funded providing coverage to a number of different populations whose members meet certain financial and resource eligibility requirements:


  • Health Families provides coverage to entire families with minor children
  • Healthy Start provides coverage to children, birth through age 19, and pregnant women 
  • Aged, Blind, and Disabled Medicaid is available to individuals who meet income and resource guidelines. Adults 65 years of age, or individuals of any age with a disability may qualify for Medicaid
  • Medicaid Premium Assistance Programs assist qualifying adults with the cost of their Medicaid expenses, including premiums, deductibles, or coinsurance
  • Medicaid Waiver Programs are for eligible consumers with chronic medical conditions or severe disability, allowing them to remain in their homes or community instead of being institutionalized
  • Nursing Facility Medicaid provides financial assistance to eligible individuals to help offset the cost of a nursing home stay
  • Expanded Medicaid began in January 2014 to provide coverage to individuals not previously eligible under Medicaid, including adults up to age 64 living at or below 138% of Federal Poverty Level (FPL)
  • Specialized Recovery Services (SRS) or Severe and Persistent Mental Illness (SPMI) programs provide Medicaid coverage to eligible adults over the age of 21 and include additional mental health services.

How to Submit Application

  • Submit Online: Applications can be submitted by visiting Self Service Portal Home Page (ohio.gov) or www.benefits.ohio.gov
  • Submit by Phone: Applications can be submitted by calling our call center at 844-640-6446 or by completing a paper application obtained by contacting the agency. Call center hours are Monday, Tuesday and Wednesday from 8:00 am – 4:00 pm, Thursday 8:00 am – 12:00 pm and Friday 8:00 am – 3:00 pm.
  • By Paper Application: Applications can be submitted by completing a paper application obtained by contacting the agency.
